Care Coordination Operations Lead | $90/hr Remote

Position: Care Coordinator

Type: Hourly contract

Compensation: $70 to $90 per hour

Location: Remote (NY, NJ, MA priority)

Commitment: Flexible hours during short term project

Role Responsibilities

• Design and operationalize a human led care coordination service across education, therapeutic, and medical domains.
• Define care coordinator responsibilities, boundaries, escalation paths, and operating principles.
• Build and refine playbooks and SOPs for intake, triage, case planning, referrals, and cross system coordination.
• Establish quality controls and performance metrics for care coordination teams.
• Collaborate cross functionally to ensure scalable, structured service delivery.

Requirements

• Experience overseeing or operating care coordination services supporting families with complex needs.
• Demonstrated ability to design operating models, scope boundaries, and documentation standards.
• Experience building workflows, playbooks, and quality assurance systems.
• Strong written communication skills and structured operational thinking.
• Ability to work independently in a remote, fast paced environment.
